A lot of information is incomplete or missing on the death entry in the Illinois Database. I suspect a poor microfilm image or mutilated document may be to blame.

The cemetery mentioned in the record as "F... View ..." was most likely Forest View Abbey Mausoleum which was allowed to go into disrepair over the years and finally torn down. Remains were re-interred in the neighboring Greenwood Cemetery which supposedly maintains all of the original records.
