  General Notes:

Was his middle name "Grey" as claimed by a few researchers or was it "Guy"? On the hand-written birth register for Williams County, Ohio, his middle name was entered as "Grey". On the 1917 World War I Draft Registration Form his middle name is written in cursive and is very much readable as "Guy" but with some imagination, this handwritten word could be read as "Grey". On the 1942 World War II Draft Registration Form his middle name is printed as "Guy".

To complicate matters, the Michigan Death Certificate was completed with his middle name as "Gary".
