General Notes:
The death certificate for his son, Cloyd Eugene Johnson, gave the full name of his parents as "Richard M. Johnson and Mary M. Fast.". This proves his given name was Richard instead of just Menter or Mentor as shown by several other researchers. In addition, his own death certificate was issued in the name "Richard M. Johnson".
As if the possibility of a previous marriage of his wife, Mary Fast, wasn't confusing enough, the death certificate for Richard Johnson adds more confusion over the names of his own parents. It shows his father, John Johnson, born in Massachusetts but inexplicably shows the name of his wife, Mary Fast, born in Ohio as his mother. I feel confident that whoever filled out this death certificate card erroneously wrote in on this line the name of Richard's wife instead of the name of Richard's mother. Supporting this conclusion, I find an 1880 US Census record in Rochester, Lorain County, Ohio for an 81 year old John Johnson from Massachusetts and his wife, 74 year old Alingra from New York. These birth locations match those that Richard stated for his parents on his 1880 US Census record.

Baughman, in 1879.1 2 (Mary Melissa Fast was born on 7 September 1853 in Ohio,1 2 4 5 died on 17 April 1917 in Troy Township, Ashland County, OH 5 and was buried on 19 April 1917 in Troy Township, Ashland County, OH 5.). The cause of her death was Indigestion.3

NOTE - Mary stated that she had 5 children but I can account for only 4 children with Richard Johnson. The fifth child must be Bertha Curry, her daughter from a previous marriage and who she listed on the 1880 census but her whereabouts are unknown. (Brian L. Lightfoot)
