  General Notes:

There is an US Passport Application filed in 1907 under the name of Carrie F. Carothers who gave her birth date as 13 Feb 1866 in Youngstown, Ohio. I'm not positive that this is the same Carrie Carothers because 15 years earlier she was married to Edward Ward in Beaver County, Pennsylvania. It is possible that they divorced and she was using her maiden name at the time.
